General Reports

Report Name

Report Description

This report provides the number of members within designated balance ranges, including the average and maximum balance in each range.

The report categorizes members into the following balance ranges:

  • $0

  • $.01-$500

  • $501-$1,000

  • $1,001-$1,500

  • $1,501-$2,000

  • $2,001-$2,500

  • $2,501-$5,000

  • Greater than $5,000

The data provided is current as of the date shown on the report.

Check the card order status for your employees. Cards are typically received 7-10 business days after mail date.

Review payments and invoices for contributions, deductions and fees. Click on an invoice ID or payment ID for more details.
Report details: invoice date, invoice ID, payment date, payment ID, invoice amount, payment type (check, ACH, wire, etc.), descriptions of invoices, and payment status.

IRS Form W9

You can request a copy of HealthEquity’s W9 from the Client Service Team.

IRS Form 5500

If HealthEquity administers your COBRA plans, the COBRA team can provide you with a report to complete the form 5500. You can contact them at 888.678.4881 or chat from your employer portal.

If you are paying fees, you can run the Invoices and Payments report to complete the necessary forms for your Schedule C tax reporting.

If you are not paying fees, you would need to consult your CPA as you may then be the responsible party to complete the tax forms. HealthEquity is not able to provide you with tax advice.

Patient-Centered Outcomes Research Institute (PCORI) Reporting

The Client Service Team can assist you in getting the information needed for PCORI.

You will be provided a report that includes the dependent information and one that doesn’t.

If you have questions on how to file PCORI you will need to speak with a tax advisor.
